The history is very important tool in the evaluation of patients with cardiovascular disease. In addition to aiding diagnosis, the history provides information about the patient's functional capacity. With many cardiac disorders, the functional capacity can be a determinant of prognosis and can play a role in determining the intensity of medical treatment or selection of surgical therapy. There are several classification schemes for functional capacity. Despite the central role of the history, it must be recognized that the cardinal symptoms are not specific for cardiac disease. Alternative causes(e.g. gastrointestinal, pulmonary) often must be excluded. Also, some patients with cardiac disease may report atypical or with no symptoms.
Although the most important function of the lung is to provide gas exchange, the lungs also serve as an important phagocytic organ, an endocrine organ, and a filter to collect emboli. The epithelial lining of the lungs,ike the skin, is exposed to the environment; thus, environment injuries are commonly expressed as respiratory disorders. The lungs serve as a paramount role in regulating blood PH, and the lungs and upper airways serve to warm and humidify inhaled air. Heat and water loss, therefore, are constants of lung function, and disease and injury to the lung usually affect several of these functions since they are interrelated. To arrive at the correct diagnosis of a pulmonary disorders, these interwining roles must be considered. Of all the information the clinician may collect in coming to a decision regarding diagnosis and treatment, a careful history provides the most important data.
