呵呵~在PolyU的PBL课程结束了。讨论了三个Case，护理方向的。每个case耗时四周，第一周分析讨论，第二周小组PPT presentation，第三周确定个人计划，第四周完成1500字的作业，要求是提出三个干预设想，并用一系列的知识来证明和推断该假设的可行性、合理性以及预期成果，每篇不少于8个五年之内的来源可靠的reference。
Pbl课程的分数占本学期每门课程的30%，至关重要，从大一开始到大三，每个学期都有。我的属于一年级的内容，每个病例都有生理和心理的方面需要分析。
第一个是关于高血压的病例，不是很难，只是要基于患者的家庭和工作情况确定相应的计划。
第二个是关于儿童骨折和单亲家庭，具体如下
One afternoon Ms. Chan, a 40-year-old woman, brings her 7-year-old son Ah Ming
into the emergency room. He says that he fell while walking down an escalator
earlier that morning. Ms. Chan speaks very rapidly and seems terribly afraid about
her son’s health. Ah Ming is crying uncontrollably and there is blood running
down his face. The doctor’s examination reveals that Ah Ming has broken a bone
in his right forearm and has a facial abrasion. Ah Ming remarks that this happened
because he was not paying attention. He stresses that it wasn’t his fault. He didn’t
tell his mother immediately because he was afraid she would scold him. Ms. Chan
has been divorced recently and is now working on a low pay job as a cleaner to
support the family. She feels very discouraged and doesn’t know what to do. Also,
she feels very helpless because she has to stay with her son in the hospital, and she
is not sure how long it will take. You are asked to admit him to the ward.
第三个则是关于老人疥疮和老年痴呆症的病例
Shui Kam is a 70-year-old woman who is admitted to your medical ward.
She was brought to the hospital by a social worker who visits her
regularly. Her chief concern is itching all over her body, which is most
severe at night. She has been treating herself with some Chinese herbs
from the herbal shop, and the condition is getting worse. She lives alone
in a public housing estate. She has a medical history of mild dementia
and sometimes wanders in the street during the night in only her
nightclothes. She also likes to collect tins and rubbish, which she stores
in her flat.
On admission, she is sitting in a wheel chair, and she is accompanied by a
health care assistant (HCA). The HCA reports to you that she is not very
cooperative and walks with an unsteady gait. On examination, she is
skinny and smelly. Her clothes are dirty. She also has multiple
excoriations on her extremities, hands, abdomen, back, and neck.
After physical examination by the doctor, Shui Kam is diagnosed with
scabies. The doctor prescribed lotions to treat her.
When you go to apply the first lotion treatment you find that Shui Kam
has changed into her own clothes and that she is preparing to leave.
When you tell her that she needs the lotion to treat the itching and that it
is better for her to stay in hospital, she yells at you “I don’t like the lotion.
I don’t want to stay here. I have to go! I have to go!”
